Record effort, additional inputs and the outcome: recovered, pending or lost. You need the original order, lot or serial where applicable, held quantity and location. Describe the observed defect and who authorized an attempt at recovery. Work may use another operation, technician, tool and material; those differences justify a separate record. Reopening an order and changing its earlier result without explanation is insufficient: it obscures how much passed first time and how much required additional work. The behavior you need must become tests using your actual units and records. ## Example: one hundred manufactured, not one hundred ten **Synthetic orders OR100 and RW10** OR100 makes 100 units: 90 pass first inspection and 10 are held. RW10 works on those same 10, adds 4 caps and 0.20 kg adhesive, recovers 8 and loses 2. Accepting eight recovered units increases accepted stock from ninety to ninety-eight. The two lost units have their own disposition. Four caps and adhesive are additional consumption of other materials and must leave their respective stock once. They cannot be added to the original unit count to suggest higher production. ## Does technician completion mean acceptance? Separate work completed from inspection accepted when your process requires it. The technician may finish an operation while quality has yet to release the lot. Retain the observation, applicable test and decision owner. If only six of the ten are recovered and two remain pending, partial closure must show six recovered, two lost and two held; it cannot release everything because a task appears complete. This helps relate the two irrecoverable units but does not prove physical removal or specify how to repair them. Rework suitability and safety need a technical decision beyond a generic button. ## How do you show first-pass quality and the final outcome? Keep 90 out of 100 accepted on the first attempt and 98 out of 100 accepted after rework. They are different results. Extra effort must remain available by order and defect even if the required goods are eventually delivered. A second recovery attempt needs a link to the earlier one and justification; do not overwrite the first report or assume repeated repair is always technically valid. ## What tests should precede system acceptance? - Normal: RW10 identifies the ten held units and its OR100 relationship; the final result is 98 accepted and 2 lost. - Exception: units from another lot or already released cannot silently enter this rework. - Partial: completed work without inspection retains the agreed state; pending units remain visible. - Retry: repeated C10 keeps one closure, eight releases and one issue of additional inputs. - History: the original defect and ninety first-pass accepted units remain retrievable after recovering eight. ## What evidence should you bring to a diagnosis? Not in this case: they are the same previously manufactured, held units. Their state or location changes; recovery releases accepted units without duplication. ### Does extra consumption belong to the original order? It may be consolidated for analysis, but retain its rework record, units, quantities and relationship. Financial allocation needs specific rules. ### Can I erase the rejection once the unit is fixed? Keep the initial and final results. Recovery does not erase the need for inspection and additional work. ### How do I close incomplete rework? Record accepted, lost and pending quantities separately.
